1 june 2011 - Katowice, ul. Chorzowska. Chwilę wcześniej skład ten spowodował ok. 10 minutowe zatrzymanie, spowodowane awarią drzwi zarówno w pierwszym, jak i drugim wagonie i problemami z ruszeniem. Po naprawie wagony ruszyły w dalszą drogę. W zatrzymaniu na chwilę utknęły wagon #806 na 6 i #551 na 23.
